PAK-SEHAT: Advancing Preventive Cardiology Through Local Evidence

Cardiovascular disease (CVD) remains a major health challenge in Pakistan, with an increasing burden among younger populations. Recognizing the need for locally generated evidence, PAK-SEHAT (Pakistan Study of Premature Coronary Atherosclerosis in Young Adults) was launched as a landmark 10 year longitudinal research study to better understand premature coronary atherosclerosis and cardiometabolic risk in Pakistan.

The study is led by Dr. Bashir Hanif (Principal Investigator, PAK-SEHAT) in collaboration with Getz Pharma. The study is building a real-world evidence repository that currently does not exist in Pakistan and is rare even across South Asia. By generating local evidence, PAK-SEHAT aims to improve understanding of cardiovascular risk patterns, disease hotspots, and demographic trends, supporting more informed approaches to prevention and healthcare planning.

A major milestone was achieved in 2024 with the completion of the baseline phase, during which 2,000 participants were enrolled across seven cities in Pakistan. Comprehensive assessments were conducted to establish cardiovascular health profiles and create the country’s first longitudinal dataset focused on premature coronary atherosclerosis. In addition, more than 1,500 DNA samples were collected and stored, creating opportunities for future genetic research and advancements in precision medicine.

As the study enters its first follow-up phase, where participants will be reassessed to evaluate changes in cardiovascular risk factors, monitor the progression of subclinical coronary atherosclerosis, and document major adverse cardiovascular events. This longitudinal follow-up will provide valuable insights into the evolution of cardiovascular risk within the Pakistani population and contribute to evidence-based prevention strategies.

Beyond research, PAK-SEHAT has the potential to strengthen cardiovascular risk assessment, screening, and preventive care by identifying early markers and risk factors associated with atherosclerotic cardiovascular disease (ASCVD). Findings from the study are also expected to contribute to scientific knowledge through publications, collaborations, and broader dissemination initiatives. Through the collective efforts of investigators, research partners, and Getz Pharma, PAK-SEHAT is helping advance preventive cardiology in Pakistan. By generating robust local evidence and deepening understanding of premature coronary atherosclerosis, the study is contributing to a stronger foundation for future cardiovascular health and research in the country.
